    Avail the free registration of CHIEF MINISTER HEALTH INSURANCE SCHEME (CMHIS) Card at e-SEWA KENDRA located at Tuensang District Court

    Documents Required for:

    Employees:
    i) PIMs details
    ii) PAN CARD / Aadhar card
    iii) Mobile number (preferably mobile number linked with Aadhaar)
    Note 1: If both spouses are government employees, the same must be declared in the application form, and will be considered together as a
    single family.

    Supporting documents for dependents while registering:
    i) Proof of age of offspring’s.
    ii) Brothers/sisters – Aadhaar / Birth Certificate / HSLC Admit Card or any other valid Govt. issued Id document
    which mentions the relationship of dependent with the government employee.
    iii) Birth Certificate issued more than a year after birth should self attested by the parent(employee) if not appended with the magistrate
    order as per below provisions.

    Ration Card Holders:
    i) Ration Card Holders can get Ayushman CMHIS Card issued against their ration card without going through the registration process in this
    portal.

    Pensioners:
    i) PPO no.
    ii) PPO(Pension Payment Order) 1st, 2nd and 3rd page xerox copy.
    iii) email / mobile number (preferably mobile number linked with Aadhaar).

    New Registration:
    i) Indigenous Certificate / Permanent Residential Certificate / Aadhar card / Ration card / PAN card / email / mobile number (preferably mobile number linked with Aadhaar).
    ii) System generated “Self Declaration Form”.

    NB:
    Indigenous certificate / PRC must be issued after 2016.
    Name should be same as Aadhaar.
    Details once entered cannot be change. Incase of details mismatch with Aadhaar, kindly update Aadhaar.
    Households already covered under PM-JAY are not eligible.
